Please complete our NCVC Participation Waiver by filling in the contact info and electronically signing the waiver agreements. Once the waiver is submitted, we will keep it on file and you will not need to complete it again unless asked.
This is not clinic or lesson registration. It is only a Participation Waiver that we need to have on file when you do not use online registration for clinics, lessons or other NCVC event. It is not necessary to complete prior to every event - just prior to your first clinic or lesson or when your contact info changes.
